Provision of timely and professional integrated support in a home environment for persons with disabilities and elderly people dependent on care. Expanding the possibilities for providing social and integrated health and social services in a home environment. Provision of hourly mobile integrated health and social services at homes and psychological support for elderly people unable to care for themselves and persons with disabilities. The project also foresees the inclusion of services for the delivery of food, food products and products of first necessity, performing urgent administrative and household services with the funds of the users. Mobile services include a variety of social, health, administrative, household activities, as well as activities aimed at improving the quality of life of the elderly and persons with disabilities, depending on the specific condition and limitations. Services will be provided on an hourly or on-demand basis of 80 users who are informed and agreed to participate in needs assessment and support planning. The specific activities are aimed at stimulating the user's activity, taking care of health, limiting the risks at home and the risk of dependence on institutional type of care. During the implementation of the project, supervision and training of staff providing services in the home environment will be provided.
