The project proposal is aimed at providing mobile integrated health and social services to adults unable to care for themselves and people with disabilities, which will improve the quality of life and access to social services in response to complex needs, incl. and health, aimed at overcoming the effects of social exclusion and poverty. It is planned to hire a mobile team that will provide integrated health and social services at homes and deliver food, foodstuffs and basic necessities, pay household bills, request and receive urgent administrative and household services (with funds from users) to at least 163 users. The services will be provided on an hourly basis, within 20 months, based on the identification needs of the target group, as a result of an assessment of the person's individual requirements and a selection made. According to the project, the persons from the target group will be provided with information on prevention and prophylaxis in view of health and socially significant diseases, including consultations with specialists if necessary. Another main activity is organizing and conducting supervision and training of employees providing hourly mobile integrated health and social services in a home environment and providing psychological support to elderly people unable to care for themselves and persons with education. The "Home care in Krumovgrad Municipality" project will help to ensure adequate social protection, overcome social exclusion, improve social accessibility and re-acceptance by improving access to integrated social services.
