The overall objective of the project "Home care in the municipality of Kardzhali" is support in a home environment aimed at persons with disabilities and elderly people dependent on care. The specific objectives of the project proposal are: improving equal and timely access to quality and sustainable services for people with disabilities and elderly people, unable to provide self-care, dependent on care in the territory of the municipality of Kardzhali; providing hourly mobile integrated health and social services in a home environment to people with disabilities and elderly people dependent on care; improving accessibility, including for people with disabilities, the efficiency and sustainability of health systems and long-term care services. The current project will build on the implemented activities and the achieved results of the other patronage care projects. The target group of the project includes 355 inhabitants of the municipality of Kardzhali - adults, incapable of self-care and people with disabilities and employees of providers of social and integrated health and social services in the municipality of Kardzhali. The main activities of the project are related to selection of potential users, selection and training of a management team and staff to deliver the services, provision of hourly mobile integrated health and social services and psychological support in a home environment, delivery of food, foodstuffs and products of basic necessities, paying household bills, applying for and receiving urgent administrative and household services (with consumer funds), as well as providing the persons of the target group with information on prevention and prevention of health and socially significant diseases, including consultations with a nutritionist and other specialists if necessary within 21 months. The implementation of the project will contribute to improving the quality of life and long-term care of the representatives of the target group.
