The project proposal of Kyustendil Municipality envisages the provision of social and integrated health and social services for persons with disabilities and elderly people dependent on care in a home environment, incl.and in the context of pandemic situations.The activities will cover a minimum of 331 persons, representatives of the target group-adults unable to care for themselves and people with disabilities. The current project will build on the implemented activities and the results achieved under the successfully implemented projects of the municipality under the procedures "Patronage care for the elderly and persons with disabilities"and "Patronage care +" and for the persons with disabilities and the elderly, support will be provided through the implementation of the following activities: 1.Provision of hourly mobile integrated health and social services at homes and psychological support;2.Delivery of food, food products and basic necessities, payment of household bills, requesting and receiving urgent administrative and household services with consumer funds;3.Providing the persons from the target group with information on prevention and prevention of health and socially significant diseases, including consultations with a nutritionist, etc. specialists if necessary. The implementation of these activities will be realized by the "Patronage Care Center" created in 2019 at the Complex for Social Services-Kyustendil and by forming a team of specialists who will provide social and integrated health and social services depending on the individual needs of the persons identified on the basis of a needs assessment.The appointed persons will be supported by providing the necessary training for the performance of their activities,as well as by conducting regular individual and/or group supervisions, psychological support and counseling.The provision of services will be assisted by providing transport for staff providing the services to/from individuals' homes.
