The project proposal "Care at Home in Lukovit municipality" will provide assistance and support to at least 97 people with disabilities and elderly who cannot self-assist, living on the municipality's territory. The project will expand the opportunities of the patronage care through hourly mobile integrated health and social services in the home of people in need, depending on their identified needs. The team providing the new type of service will include 30 home assistants, nurses, a rehabilitation therapist, a psychiatrist, social workers specialized in working with people with severe disabilities, psychologists, as well as a dispatcher and a driver. We foresee training and supervision for the staff - initial for the new entrants and upgrading for the others. Services will be provided on a mobile, scheduled basis. For this purpose, a motor vehicle purchased under the "Patronage care for the elderly and disabled" will be used, which will be necessary for the quick transport of the service team and will allow its provision in all the settlements of the municipality, including remote areas away from the municipal centre. The persons from the target group will be provided with information from the health experts regarding the prevention and prevention of health and socially significant diseases, and if necessary, consultations with other specialists will be provided. The project will fund the purchase of work clothing, including personal protective equipment for the staff of the service, occupational health costs, and materials and supplies for medical care. The project duration is 24 months, and the hourly mobile integrated services will be provided for 22 months.
