The current project proposal includes the execution of activities that provide a set of part-time mobile integrated health and social services to the most vulnerable groups of the municipality. The services represent one service of common economic interest and through an assignment deed the Mayor of Pavlikeni municipality will grant its execution to Domestic Social Patronage – Pavlikeni. Team for management of the services will be included in the project, who will put an accent to health and psychological support of the users, besides the daily care. The home service comprises a complex of activities like provision of part-time mobile integrated health and social services, supply of food and goods of first necessity, provision of information about health issues prevention, medical services for prevention of health deterioration, mental health support and rehabilitation services. The services will be provided on the basis of identification of the needs of each user and they will be complied with their personal choice. The assessment of the users’ necessities will be made by a multidisciplinary team. The service will be provided for the period of 25 months to 166 users minimum, who are representatives of target group under the procedure “Elderly unable to care for themselves and disabled people”. In the scope of the project supervision and training will be provided for the staff members who execute the part-time mobile integrated health and social services in domestic environment in compliance with the provisions of the Ordinance on the quality of social services. The current project proposal will upgrade the implemented social projects of Pavlikeni municipality under OP “DHR” through 2014 – 2020 programme period and the quality of the provided services for the users will be improved.
