The project will respond to needs that are identified in Pernik Municipality for social and health support at home, for people who, due to their health and age, are unable to look after oneselfs and are dependent. The main purpose of the project is to provide support in home environment for people with disabilities and elderly people that are dependent on care, by providing social and integrated health and social services in home environment, including in term of a pandemic situation. The activity that will be implemented is "Homecare in the municipality of Pernik", including the following sub-activities: 1. Part-time mobile integrated health and social services at home and psychological help will be provided for eldery people unable to look after oneself and people with disabilities; 2. Food, food supplies and essential products will be delievered. Paying household bills, requesting and receiving urgent administrative and household services (with user funds) are also allowed; 3. Providing the persons from the target group with information on prevention and prevention of health and socially significant diseases, including consultations with a nutritionist, etc. specialists when needed; 4. Providing supervision and training to emploies that provides hourly mobile integrated health and social care services in home environment. The target group are eldery people unable to look after oneself and people with disabilities, including people with permanent impairments; limited motor capabilities; eldery people with high risk of social isolation; people at risk of institutionalization due to inability to care for themselves. As a result of the implementation of the project , 470 adults people unable to look after oneselfs and people with disabilities will recieve hourly mobile integrated health and social services as homecare and 92 will be engaged in the provision of the services.
