This project contributes to the implementation of Specific Recommendation 4 (2019) and 2 (2020), as well as Principle 18 "Long-term care" from the European Pillar of Social Rights, and is aimed at providing, on the territory of the Municipality of Plovdiv, social and integrated social and health services in a home environment, including in the context of pandemic situations, for elderly people unable to care for themselves and people with disabilities. With the combined hourly mobile service "Home Care," users can receive medical and social services, psychological support, and consultations with specialists in their home environment, based on their individual needs, as well as delivery of food and products, including essentials, and information on how to prevent and treat health and socially significant diseases. For this purpose, within the project, specialists will be appointed (service management team, medical staff, psychologists, "Social activities" associates,nutritionists, maintenance workers, hairdressers etc., assigned on the basis of the identified needs of the target group and as a result of an individual needs assessment and the selection of users) who will take care of 1055 persons from the target group. Through the specialised office set up within the project, users will be provided with a permanent and direct connection with the mobile service teams, which will facilitate access and timely provision of the services. It is planned that the integrated mobile health and social service "Home Care" will be provided hourly for a period of 23 months, and within the project it will be free of charge for users.
