The project proposal "Care at home in the municipality of Rouen" is aimed at providing the mobile integrated health and social service to adults unable to care for themselves and people with disabilities, which will improve the quality of life and access to social inclusion services in response to the complex needs, incl. and health aimed at overcoming the effects of social exclusion and poverty. It is planned to hire a mobile team that will provide hourly mobile integrated health and social services at homes and will deliver food, foodstuffs and basic necessities, pay household bills, request and receive urgent administrative and household services ( with user funds) to at least 142 users. The services will be provided on an hourly basis based on the identified needs of the persons of the target group, as a result of an assessment of individual needs and selection of users within 22 months. The project envisages providing the target group with information on prevention and prevention of health and socially significant diseases, including consultations with specialists if necessary. Another main activity is organizing and conducting supervision and training of employees providing hourly mobile integrated healthcare social services in a home environment and providing psychological support to elderly people unable to care for themselves and persons with disabilities. The project will help ensure adequate social protection, overcome social exclusion, improve accessibility and social inclusion by improving access to integrated social services.
