The main objective of the project proposal is to provide support in a home environment for persons with disabilities and elderly people dependent on care and to ensure adequate social protection and access to basic services for the vulnerable groups of persons. Within the framework of the Project, elderly people, unable to care for themselves and persons with disabilities will be provided with hourly mobile integrated health and social services at their homes as well as psychological support. It will also include delivery of food, foodstuffs and basic necessities, as well as payment of household bills, requesting and receiving urgent administrative and household services (with consumer funds).The persons from the target group will be provided with information on prevention and prophylaxis of health and socially significant diseases, including consultations with a nutritionist, etc. specialists if necessary. Workers providing hourly mobile integrated health and social services in a home environment will be provided with supervision and training, as well as transport to/from consumers' homes. The provision of integrated health and social services will be for a period of 24 months. Within the framework of the Project, 341 persons from the target groups will be covered. The services will be provided on an hourly basis based on the identified needs of the individuals from the target group, as a result of an assessment of individual needs and selection of users. For the provision of integrated social and health services, after a selection, staff will be appointed, including the following positions: social worker, social activities associate, home health worker, nurse, kinesitherapist/rehabilitator, masseur, psychologist. Positions assigned to administer the service: Coordinator, accountant, technical assistant, hygienist. To guarantee transparency in the implementation of the project, information and publicity activities will be carried out.
