The project proposal is aimed at providing support in a home environment to persons with disabilities and elderly people dependent on care. The planned activities will cover a minimum of 381 users for a period of twelve months in the territory of Haskovo municipality. The representatives of the target groups will be supported through the implementation of the following activities: 1. Provision of hourly mobile integrated health and social services at homes and psychological support for elderly people unable to care for themselves and persons with disabilities. 2. Delivery of food, food products and basic necessities, as well as providing transport to the personnel providing the services from/to the homes of the individuals. Paying household bills, requesting and receiving urgent administrative and household services (with user funds) is also allowed. 3. Providing the persons from the target group with information on prevention of health and socially significant diseases, including consultations with a nutritionist or other specialists if necessary. 4. Providing supervision and training to employees providing hourly mobile integrated health and social services in a home environment. The implementation of the activities will be accomplished by forming a team of specialists - nurses, rehabilitators, psychologists, social workers and home health workers, who will provide various health and social support activities, depending on the identified needs of the persons of the target group, as a result of an assessment of the individual needs and selection of users. With the implementation of the project proposal, the possibilities for providing social and integrated health and social services in a home environment will be expanded, incl. and in the context of pandemic situations. The results achieved in other projects implemented in the municipality of Haskovo, which are financed with funds from the state budget and under the OP DHR 2014-2020, will be upgraded.
