Dem. trends such as population decline, continuing ageing, increasing health problems, as well as the high number of elderly people living alone and in need of individual support, determine the need for patronage type services. With the implementation of this project, SM will ensure the continuation and upgrading of the model established under similar projects for the provision of patronage services in a home environment for elderly people unable to provide for themselves and people with disabilities, in order to improve their social, health and psychological status (including in pandemic conditions). According to the terms of the procedure, the service "Home care" will be provided to persons in need from the target group through: - hourly mobile integrated health and social services for the users in their homes to provide support in their daily activities, according to the individual needs of each user; - delivery of food, groceries and necessities, payment of utility bills, requesting and receiving urgent administrative and household services (at the users' own expense), as well as provision of transport for the staff providing the services to/from the persons' homes; - providing information on prevention and preventive health; - training and supervision of untrained staff directly involved in service delivery. The SM will entrust the implementation of the Home Care service as an hourly mobile integrated health and social service to the Integration of People with Disabilities, Programmes and Projects Department and to district administrations that have expressed their willingness to provide the service. The service will be provided for a period of 19 months by providers. It is planned that 1,530 persons will be served in the capital for the period up to 01.06.2024 and 1,046 persons for the period after 01.06.2024. The project has been prepared in accordance with the Terms of Reference.
