The multi-year economic crisis has led to an intensification of social and economic problems in Athens and the risk of social exclusion is intensifying for a large part of its residents The project aims at the decentralised provision to Multi-Purpose Municipal Clinics of one stop shops of combined medical and social counselling services adapted to the new conditions to the residents of Athens belonging to vulnerable groups in Athens The services provided include indicatively Primary health care services including integrated and continuing health care services in care to elderly people with chronic diseases Psychosocial support to individuals or families facing problems Support to professional integration Primary prevention and health promotion Information on benefits and support entitled to Legal support Within the framework of the act, the municipal clinics of the Municipality of Athens will be upgraded to Multi-Purpose Municipal Clinics for the provision of combined health and social services
