The aim of the project is to increase social inclusion and prevent the institutionalization of vulnerable groups from the territory of the municipality through the provision of long-term care. This will be achieved by providing support and support services in everyday life to the elderly and persons with disabilities, and through prevention, promotion of change, assistance in meeting basic living needs and support to the individual, the family, in order to improve the quality of life and empower users in independently satisfying basic living needs, and their active inclusion in society. During the duration of the project, beneficiaries will be provided with day-to-day assistance in the home, enable social contact and social connections, and increase their sense of value and belonging. The project will last 36 months.
