The overall objective of the project is to continue and improve the implementation of the process of deinstitutionalization and transformation by ensuring the right of users to live in the community. In order to achieve the specific objectives related to providing support to the MROSP in the implementation of the process of deinstitutionalization and transformation of social service providers founded by the Republic of Croatia, support activities will be carried out: identification of key stakeholders in the process and development of recommendations for the purpose of cooperation, development of draft guidelines for preparation and support in the development of individual plans of service providers, support for the development of a transition monitoring mechanism, development of a work plan and support for the development of tools for the purpose of monitoring the transition from institutional to community-based care and support.
