The aim of the project is to increase the scale and quality of treatment and actively solve problems related to mental disorders and mental illnesses of inhabitants of Jelenia Góra by creating in the period IV.16-VII.16 a comprehensive and individualised aid model based on: —the provision of non-institutional services in the partnership formula, -coordination of health and social services, -support of a person experiencing mental illness at every stage of illness and recovery, -leading a range of environmental support and outpatient treatment. The project’s target group shall be: 1. Users of solutions(model) – in the project at least 10 entities (public and non-public) working for people with mental disorders or illnesses. 2. The recipients of the model-in the project 15 people with mental disorders and illnesses (including those with a disability certificate) and 10 representatives of families. Main results of the project: —prepared for testing an initial version of a comprehensive model of environmental treatment of mental disorders in Jelenia Góra – a partnership that will test and implement the developed model in Jelenia Góra Area Project activities: — analysis of health and social services operating in a given area indicating deficits of services for people with mental illnesses and disorders and lack of environmental support – participatory development and preparation for testing a comprehensive model of environmental treatment of mental disorders – consultation of the developed model with support users and local institutions responsible for social policy and health care, – establishing a partnership.
