Deinstitutionalisation with an opportunity for a good change: horizontal project

The horizontal project (PZ01) aims to ensure uniform test conditions for theInclusive Model (D00/15). Model testing is the main objective of the project. The model itself is operational in the form of a network of cooperation and coordination of support for people with experience of mental crises – Environmental Centre for Mental Health. Services are provided in a personalised approach and therapy focused on the needs, dignity and rights of the person. Direct actions in this project are aimed at building cooperation and coordination between medical, social and educational actors in the implementation of change (standards, change management) and the training of staff. The results of the tests will be included in the final report and developed standards of implementation and management and implementation of Model services.The implementation of the substantive tasks of the project, i.e. training, implementation of the Auxilio system, evaluation, management of change or prevention will be carried out in the districts of Bielany m.st. Warsaw, Wielicki, Wielicki,m. Koszalin and Koszalin County and Newtarska County.The target groups are partnerships in the areas of the project, as well as in the areas of the project. The most important results and products of the project are the support of 6 environmental mental health centres and an increase in the professional competence of people working in environmental assistance.At the end of the project, two most important products will be created: final report on testing and standards for the implementation of SCZP. In addition, 200 SCZP staff will be trained and several ICF diagnostic protocols will be established for adults and children and adolescents.
