The project assumes an increase in awareness and the degree of detection of cardiovascular diseases in women and men of working age who have reached the age of 18, with particular regard to identification of patients who are genetically burdened with FH, in the period from 1 March 2019 to 31 March 2022 in the Lesser Poland, Świętokrzyskie and Podkarpackie voivodships. Target groups:1. GP doctors, nurses, occupational doctors, staff providing support in the form of guidance in the field of diet, health education and physical activity2. persons of working age who have completed 18 years of age. (also persons with pension rights, but still active).The project assumes that the EDUCATIONS will be carried out for the MEDICAL and Personel CADE to provide TERMS IN TRADE, EDUCATION and Physical Activity. Access to training for men and women will be unlimited. Training facilities will be adapted for people with disabilities. The project involves establishing cooperation with at least 40 GHs, in which trained physicians as part of the MEDIC SERVICE SERVICES will be selected among patients who are eligible for the Programme. They will be interviewed (using checklist), general-internist research, BMI calculation and global risk stratification using PolSCORE and DLCNS forms. In situations where risk factors occur, the patient is directed to active prevention (professional guidance of staff on diet, health education and physical activity) and for a complete set of ATTENTIONS within the framework of GA. If necessary, the patient is directed to the SU in Krakow to provide specialised/cardiological PORADA, which assumes genetic diagnostics with NGS technology and cascade diagnostics of relatives from FH. The final stage is the end-to-end reading of the LOCATION with a summary and recommendations. At the same time, information and education activities will be conducted.
