The pilot implementation of the GP-level integrated care model aims at gradually developing a patient-oriented system where, instead of focusing on the provision of services, more emphasis is placed on the use of preventive tools. Such a system will be better adapted to the current situation in the health care environment and demographic processes.By means of open and transparent recruitment, actors will be selected to ensure that the model is tested in all regions of the country, both in urban and rural areas. GP facilities will be obliged to adapt the organisational structure and internal IT systems in order to manage integrated care. The adaptation process will be financed within the framework of the project, through a technological and coordination grant.The OOK Platform will be created to collect and analyse data related to the ongoing implementation of pilotage, exchange knowledge on best practices and educational activities. It will also serve as a basis for the development by the Ministry of Health through consultation of proposals for amendments to specific legal acts that will be necessary in order to implement the OOK effectively in Polish legislation. The provision of health services financed by European funds under GOD+ will include: — performing health balances for adults and conducting extensive educational activities in the field of health prevention. — management/coordination of care in 11 most popular non-infectious chronic diseases, i.a. based on an extended range of diagnostic tests and specialist consultations.The monitoring and evaluation will be carried out to verify the effectiveness of implementation of the adopted model assumptions and quality monitoring, including patient satisfaction. Training for medicinal entities participating in the integrated care project is also planned.
