The establishment of a DDOM in the ZOZ in Włodawa as a continuation of the treatment of elderly dependants through the provision of care and rehabilitation services in order to restore their efficiency and ability to function in the environment.

The aim of the project is to create the Daily House of Medical Care in the ZOZ in Włodawo. SP ZOZ in Włodawa has been providing health services on the basis of contracts concluded with the NFZ o/Lublin for many years. It has infrastructure adapted to people with mobility, kitchen and extensive diagnostic facilities and many years of experience in the medical services market. Thanks to the establishment of DDOM in the structure of the ZOZ in Włodów, patients who leave the hospital in the last 12 months will be able to benefit from a new healthcare service, which will continue to provide care, therapeutic, care and rehabilitation services outside the hospital. A new pro-quality form of care will be created that will eliminate restrictions on access to health benefits. The gap in family education needs will also be closed. The possibility of providing further nursing, rehabilitation and psychological care of the indicated group will also allow to shorten the period of hospitalisation and free up new, needy hospitalisation places. The results of the actions will improve the health situation and hamper the social and professional functioning of the target group in particular women in the Latvian district. As a result of the project, a DDOM will be established in the structures of the ZOZ in Włodzowa with 12 deinstitutionalised care sites for dependents, 100 patients will be looked after and completed the treatment, of which as much as 80 % will be satisfied with the functioning of the DDOM. Two tasks will be carried out within the project. The first one: purchase of medical equipment and equipment and the second: provision of health services to dependent persons requiring care, care, rehabilitation and continuation of treatment not as part of hospitalisation.
