The project consists of the organisation of DDOM in a newly created facility dedicated to the provision of long-term care services.Main objective of the project:Deinstitutionalisation of care for dependents through the creation of the Daily Medical Care House in the Provincial Specialist Hospital in Biała Podlaska, an alternative form of support for dependents and their families. Main results to be achieved in the framework of the project: Establishment of 15 deinstitutionalised places of care for dependent persons. You will take care of at least 60 patients. Of which 80 % of people using DDOM will positively evaluate it functionally. Target group of the project: 1. Patients requiring care, care and rehabilitation services due to their health status and continued treatment in a non-requiring hospital ward, who in the assessment of the level of self-reliance (Barthel scale) received between 40 and 65 points. The project will make up the majority (80 % of the total of 48 people) of patients over 65 years of age. Quantitatively relevant characteristics: 30 women, of which 24 years of age or over 65,30 males, of which 24 are over 65 years of age, 2. The Provincial Specialist Hospital in Biała Podlaska. Main tasks: 1. Equipment and equipment: As part of the task, the equipment necessary for the operation of the Daily Medical Care House will be purchased. The purchase will be made under an open tender procedure. 2. Running a day care home: The action will provide patients with appropriate health and needs assistance to improve their independence and administrative support for the project. 3. Educational activities: As part of the DDOM activities, educational activities will be carried out to the families and caregivers of the patient preparing them to continue care of the dependent person.
