Regional Centre for Coordinated Care – Development of integrated and environmental care by investments in POZ and AOS in the Boleslawieckie, Lubany, Lviv, Jelenio Góra and Zgorzelec districts until 2018

The project will last in the period 01.12.2016-30.11.2018. The project is attended by 5 partners: WS-SP ZOZ in Zgorzelec,NZOZ “Family” Elżbieta Pyrzanowska,Autonomous Public Health Care Department in Nowogrodźec,“Familia” s.c. Ally Nosiru Danesi, Olga Mazurek, Danuta Grelińska, LEKARSKA “Fons VITAE”. The target group of the project are patients with diagnosed chronic disease and pregnant women from districts where the project will be implemented. The target group is also the employees of entities participating in partnership cooperation (as follows: doctors, nurses, etc.) and non-governmental organisations promoting health and care for the elderly. The main objective of the project is to improve the availability, efficiency and quality of healthcare by 2018 in the above-mentioned districts through infrastructure investments in POZ and AOS, development of integrated and environmental care. The specific objectives shall be: 1. Implementation of the integrated outpatient health care system through the creation of a network of cooperation of the Regional Centre for Coordinated Care (RCOK), the creation of an organisational model and quality control and monitoring; 2. Shifting emphasis from services requiring hospitalisation to GP and AOS and reducing the number of hospitalisations; 3. Increasing the availability of integrated IT tools to support integrated care systems; 4. Modernisation and construction of facilities that will meet the requirements for the creation of RCOK by the end of 2018; 5. Upgrade the network of RCOK facilities with medical equipment by the end of 2018; 6. Reduce the financial burden on patients by reducing unnecessary expenditure on medicines, access to specialists and treatment in private doctors. 7.
