The project will consist of improving access to specialised care services at the place of residence for dependent persons due to disability or illness, in particular for children and adolescents up to 30 in the field of physiotherapy/rehabilitation (30 persons). The project also covers the environment of these people in the field of psychologist support and dietetics (30 persons) (parent/carer of a real person).The project aims at preventing social exclusion, isolation from family and local community through individualised scope of support. Specialist services will be provided at the place of residence of the beneficiary (access to the disabled person).A dietetician will be able to use psychological advice and consultation in the GOPS building in Rogowo, which is adapted to people with mobility disabilities. The service provided by the physiotherapist and the rehabilitant will be provided at the place of residence.The support of psychologist and nutritionist will benefit people from the environment at the GOPS headquarters.The services in question will be of general interest.As part of the project, the project will create 2 place to provide SUO in the place of residence, which will benefit 30 people. (less, disease) and 30 people around them (to the extent necessary to support them). As a result, 2 places of service to be provided to the community will be supported after the completion of the project. Physical rehabilitation and improvement of impaired functions of the organism take place within the scope of the Act of 27 August 2004 on health care services financed by public funds (Dz. U. No. 210, item. 2135, as amended) according to medical advice or specialist in the field of motor rehabilitation or physiotherapy.
